Oral health professionals are now treating and will continue to treat more older adults than ever before in history. This program will focus on common physical and physiologic changes associated with aging and their impact on the delivery of care, as well as a discussion of useful communication, behavioral and practice management approaches appropriate for the elderly. With knowledge, flexibility and willingness, working with older adults can be a very rewarding opportunity.

Dr. Janet Yellowitz began her career as a dental hygienist, graduating from Forsyth School for Dental Hygienists. After completing a Masters in Public Health with a specialty in Gerontology and Geriatrics at the University of Minnesota, she completed her dental training at the University of Pennsylvania School of Dental Medicine.

Dr. Yellowitz's interest in working with older adults began as a faulty member at Forsyth, where she developed collaborative programs with Harvard's Dental School to provide oral health services to local long term care facilities. Since 18991, she has been the Director of Geriatric Dental Programs at the University of Maryland, School of Dentistry. In this capacity, she provides didactic and clinical training to dental and dental hygiene students in the areas of Geriatrics and Gerontology.

She has been the Dental Director at a local long term care facility for the past 17 years, and is the Past President of the Special Care Dentistry Association and American Society for Geriatric Dentistry, a component of Special Care Dentistry Association.

Course Title:    An Overview of the Current Status of Oral Cancer
Presenter:      Robert  A. Ord, DDS, MD, FRCS, FACS, MS
1.         Changing Demographics of Oral Cancer (HPV and all that)
2.         Oral Cancer Screening and Premalignant Lesions  (including CDX Vizlite, etc.)
3.         Reconstruction of the Jaws

Course Description:
The Oral Cancer section will review 3 topics; demographics, screening and changes in the techniques of jaw reconstruction.  In the last 20 years there has been an increasing change in the demographics of oral cancer.  Although the stereotypical patient with oral cancer is an older male who abuses tobacco and alcohol, there has been a marked increase in female patients, young patients under the age of 40 years and patients with no risk factors i.e. non-smokers.  The reasons for these changes and the impact they have on the prognosis for oral cancer in these cohorts will be discussed.  The recent recognition of the role of Human Papilloma Virus (HPV) particularly in oropharyngeal  cancer will also be emphasized.
The prognosis of oral cancer is related to the stage of diagnosis and early stage cancer has an improved survival with less patient morbidity.  This is the rationale for oral cancer screening routinely on all patients over the age of 18 years.  The technique of oral cancer screening and the identification and management of pre-malignant conditions will be emphasized.  The evidence for the use of adjuvant techniques commonly being marketed to the oral health care professional will be critically reviewed.
In the final section the developments that have changed the way in which surgeons undertake mandibular reconstruction in oncology patients are presented.  In particular the introduction of technology allowing pre-operative computerized planning combined with the surgical techniques of Microvascular surgery have revolutionized the defects we are able to restore and improve functional and esthetic outcomes.

Dr. Ord is board certified and maintains a clinical focus in surgical oncology of the head and neck with an additional interest in benign tumors of this region and reconstruction of the jaws and oral cavity.

1.  Temporomandibular joint disorders: non-surgical and surgical approaches.

This session will outline the management of temporomandibular joint disorders, from both a non-surgical and surgical perspective.  The session will provide an anatomic and functional review of the TMJ, an understanding of the most common etiologies, and a strategic approach to the diagnosis and management of TMJ disorders.  State of the art office based management will be discussed as well as more advanced surgical techniques.

Objectives:

  1. Develop an understanding of TMJ disorders, with a systematic approach to evaluation, diagnosis and management.
  2. Understand the appropriate selection of surgical candidates for TMJ surgery.

2.   Management of patients taking bisphosphonate medications and how this relates to dental patients and implants.

The number of patients taking bisphosphanate medications continues to increase.  Although there are tremendous advantages to these medications for many patients, they carry the risk of osteonecrosis of the jaws.  This session will review bisphosphonates and their medical uses and focus on management of these patients in the dental office, including those in need of surgery and implants.  The latest guidelines of care will be reviewed.

Objectives:

  1. Understand the risks of managing patients taking both oral and IV bisphosphonates.
  2. Become familiar with the latest treatment guidelines and management of patients taking these medications.
  3. Review some complex clinical cases involving bisphosphonate medications.

Dr. Warburton trained in Oral & Maxillofacial Surgery in the United Kingdom, and completed his training at the University of Maryland Medical Center and the R Adams Cowley Shock Trauma Center.
Prior to coming to the University of Maryland Medical Center, he earned his dental surgery and medical degrees from Manchester University in the United Kingdom, where he obtained numerous academic honors and awards including the Alpha Omega Prize for Clinical Excellence. He is a Fellow of the American College of Surgeons and a Fellow of the Faculty of Dental Surgery of the Royal College of Surgeons of England. In addition to his Oral & Maxillofacial Surgery training on both sides of the Atlantic Ocean, he has completed a Fellowship in Clinical Research at the National Institutes of Health, and has numerous publications. He holds a faculty appointment at the University of Maryland Medical Center, where he is Residency Program Director, and he is the Chief of Maxillofacial Trauma at Sinai Hospital. He is a member of the American Association of Oral & Maxillofacial Surgeons, the Maryland Society of Oral & Maxillofacial Surgeons, the American Society of Temporomandibular Joint Surgeons, the American Academy of Cosmetic Surgery and the Academy of Osseointegration.
Dr. Warburton's clinical interests include facial cosmetic surgery, dental implantology, orthognathic surgery, maxillofacial trauma, and the surgical management of temporomandibular joint disease.
